Electroconvulsive therapy (ECT) versus sham ECT for depression: do study limitations invalidate the evidence (and mean we should stop using ECT)?

چکیده
SUMMARY Electroconvulsive therapy (ECT) for depression is a controversial treatment with highly polarised views about the balance between therapeutic benefits and adverse effects. Studies investigating whether ECT more effective than placebo started in 1950s, most important randomised controlled trials carried out four decades ago which was compared sham (SECT) involving anaesthesia but no electrically induced seizure. Subsequently data have been pooled number of meta-analyses found that an treatment. However, recent review quality SECT-controlled studies, based on them, concludes their too poor to allow assessment efficacy that, given its risks (permanent memory loss death), use should be suspended. This commentary critically discusses methodology this conclusions.

The sham ECT literature: implications for consent to ECT.
The author reviewed the placebo-controlled literature on electroconvulsive therapy (ECT) for depression. No study demonstrated a significant difference between real and placebo (sham) ECT at 1 month posttreatment. Many studies failed to find a difference between real and sham ECT even during the period of treatment. Claims in textbooks and review articles that ECT is effective are not consisten...
متن کاملElectroconvulsive therapy (ECT) for intractable depression following epilepsy neurosurgery
Psychopathology following epilepsy neurosurgery is a significant risk. Treatment modalities have not been addressed in the literature. As disproportionately elevated suicide rates have been reported, it is critical to treat aggressively any psychiatric illness wherein suicidal ideation is a key component. This case reports the safe utilization of electroconvulsive therapy (ECT) for intractable ...
متن کاملThe Origins of Electroconvulsive Therapy (ECT).
Prior to the 1930s, the prime mode of treatment for psychiatric outpatients was psychoanalysis. Little could be done for inpatients, other than provide sedation and social support. In the 1930s, four major somatotherapies, all interventionist in technique, were developed: insulin coma therapy, Metrazol convulsive therapy, lobotomy (psychosurgery), and electroconvulsive therapy (ECT), the only o...
متن کاملElectroconvulsive therapy (ECT) from the patient's perspective
This is a response to Dr Charlotte Rosalind Blease's paper 'Electroconvulsive Therapy (ECT), the Placebo Effect and Informed Consent', written by Julie K. Hersh who has had ECT. Hersh argues that placebo effect is impossible to prove without endangering the lives of participants in the study. In addition, informing potential ECT patients of unproven placebo effect could discourage patients from...
متن کاملSeizure threshold in electroconvulsive therapy (ECT) II. The anticonvulsant effect of ECT.
To measure the anticonvulsant effects of a course of electroconvulsive therapy (ECT), we used a flexible stimulus dosage titration procedure to estimate seizure threshold at the first and sixth ECT treatments in 62 patients with depression who were undergoing a course of brief pulse, constant current ECT given at moderately suprathreshold stimulus intensity. Seizure threshold increased by appro...
